Job summary

Elemental Management Group seeks an MDS Nurse supervised by the Director of Nursing and MDS Coordinator. The role focuses on completing MDS+ assessments, coordinating with disciplines, and ensuring timely submission to CMS. No on-calls, no weekends, and no floating to units.

The ideal candidate is a NYS-licensed RN, with RAC preferred. Candidates should have strong communication skills and basic computer proficiency for documenting and coordinating care.

Qualifications

• Basic computer knowledge required.
• Strong oral and written English communication.
• Registered Professional Nurse with NYS license.
• RAC certification is highly preferred, training provided.
• Long Term Care experience is a plus.

Responsibilities

• Develops a schedule prescribed by regulation for the IDCP team to complete timely and accurate assessments.
• Completes MDS sections using residents records, family input, care staff and physicians.
• Attends weekly Medicare meetings and presents patient status and discharge planning needs.
• Acts as liaison among rehab, center staff, physicians and families with professionalism.
• Ensures timely completion and submission of MDS and related forms to CMS.
