Complete Care, Inc. in Wisconsin is seeking an experienced MDS Coordinator (RN) to ensure accurate resident assessments and care plans while meeting regulatory and reimbursement requirements.

This role emphasizes thorough data review, collaboration with the interdisciplinary team, and timely MDS transmission. You will guide assessment completion, schedule conferences, train staff, maintain records, and stay current with MDS guidelines.

Formación

• RN license required.
• MDS experience required.
• Strong knowledge of Case‑Mix, Medicare PPS and medical reimbursement.
• Ability to work independently or in a team with minimal supervision.
• Proficient with Electronic Health Records (EHR).
• Experience in long-term care preferred but not required.
• Excellent attention to detail and organizational skills.
• Strong communication and interpersonal skills for collaboration with staff and residents' families.

Responsabilidades

• Monitor and guide completion of resident assessments in a timely manner, ensuring accuracy and regulatory compliance.
• Supervise input of MDS assessments and review data for integrity.
• Transmit MDS assessments to the state and obtain validation receipts.
• Schedule and lead quarterly/annual care plan conferences with residents, families, and the interdisciplinary team.
• Collaborate with the team to ensure documentation supports the MDS process.
• Serve as a resource for staff on the MDS process and provide training.
• Maintain comprehensive records of MDS assessments and care plans.
• Stay updated on changes to MDS regulations and guidelines.
• Perform other duties as assigned by DON and Administrator.
