Job summary

Providence Park Rehabilitation & HealthCare Center in Mobile, AL is seeking an experienced MDS Nurse Coordinator to coordinate assessments, document outcomes and help develop Comprehensive Care Plans for long-term and short-term residents.

The ideal candidate is a licensed RN or LPN with at least 1 year of MDS experience, strong communication skills with elderly patients, and a commitment to quality post-acute care.

Qualifications

• Must be a Registered or Licensed Nurse in good standing and currently licensed by the State of Alabama.
• 1 year of MDS experience required.
• Comfort and positive attitude in communicating with the elderly with multiple nursing care needs.

Responsibilities

• Coordinate and document assessments of residents.
• Assist in developing Comprehensive Care Plans per facility, state and federal guidelines.
• Monitor residents to ensure care plans are followed and updated as needed.
