Job summary

NHC HealthCare Anniston is seeking an MDS Coordinator RN to join our care team. You will work closely with residents, families, and an interdisciplinary team to complete accurate assessments and develop timely, individualized care plans.

Responsibilities include conducting and documenting resident assessments, coordinating with departments to meet deadlines, and participating in on-call coverage as needed. A valid RN license is required and prior MDS/PDPM experience is preferred.

Qualifications

• Current Registered Nurse (RN) license required.
• Strong communication, organization, and time-management skills.
• Caring, compassionate, and dependable.
• Positive attitude and team-oriented approach.
• Previous RN experience preferred.
• MDS and PDPM experience preferred.
• RAC or CMAC certification preferred.

Responsibilities

• Conduct and document accurate resident assessments
• Interview residents and family members to gather important care information
• Collaborate with the interdisciplinary team to ensure resident assessment protocols and care plans are completed on time
• Maintain the MDS calendar and help ensure all departments meet required completion deadlines
• Participate in on-call responsibilities as needed
