Job summary

Beaumonthc is seeking an experienced MDS Nurse in Indiana to ensure accurate assessment coding and up-to-date care plans for residents. You will collaborate with the RN Coordinator and establish reimbursement rates and quality metrics for public reporting.

Responsibilities include coordinating MDS assessments across admission, readmission, quarterly, and annual cycles, reviewing documentation, and leading staff training to ensure care plans are complete and compliant.

Qualifications

• Must have capacity to perform routine job duties.
• Working knowledge of nursing services, nursing administration, rehabilitation, general nursing, geriatric nursing, the MDS, and its computer application (EMR).
• Must possess strong managerial, organizational, planning and interpersonal skills.

Responsibilities

• Coordinate completion of the MDS at initial admission, readmission, quarterly, annual, significant change, and PPS/Insurance Reimbursement.
• Provide calendar monthly and ad-hoc schedule for MDS assessments; open MDS assessments in the EMR for IDT use.
• Ensure timely completion of each MDS section by each department and sign-off in EMR.
• Collaborate with departments to address problems in MDS sections and arrange re-education as needed.
• Review ICD-10 diagnosis coding and sequencing with each MDS assessment and complete Physician Queries.
• Conduct daily review of clinical records to capture highest level of care on OBRA and PPS assessments.
• Lead daily MDS-therapy meetings and weekly Utilization Review meetings.
• Participate in pre-billing review of claims per facility guidelines.
• Monitor MDS-based Quality Measures for coding accuracy and assist IDT in QAPI efforts.
• Support staff in EMR charting and documentation to meet Care Plan goals.
