LiveWell Homecare Agency, LLC is seeking an experienced MDS Nurse for the day shift with on-call rotation. You will coordinate assessments, ensure timely transmission, and support care planning aligned with regulatory standards in long-term and post-acute care.

The role requires an Ohio RN license, BSN preferred, and 3+ years of MDS experience (or 3 years general nursing if no MDS background). Join a nonprofit-founded provider delivering person-centered senior care across options.

Qualifications

• BSN preferred.
• Current Ohio RN license.
• 3+ years MDS experience in long-term or post-acute care.
• For RN without MDS experience, 3 years general nursing experience required.

Responsibilities

• Coordinate development, completion, and transmission of assessments per regulatory standards.
• Review and manage reimbursement.
• Case manage care plans and rehabilitative programs.
• Demonstrate knowledge of clinical standards, regulations, and reimbursement in long-term and Medicare care.
