Brittany Manor in Midland, MI is seeking an experienced Care Management Nurse (MDS Nurse) to remain clinically involved in patient care, coordinating MDS assessments and care plans. You will monitor RAI processes, PPS scheduling, and collaborate with physicians, nursing, and social services to ensure high-quality resident care.

Formación

• Registered Nurse, RN
• Knowledge of the Resident Assessment Instrument (RAI) process, including PPS principles
• Experience as an MDS Nurse

Responsabilidades

• Completes the MDS, CAA's and care plans within regulated time frames
• Assesses resident through physical assessment, interview and chart review
• Discusses resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff
• Reviews information from hospitals, consults, and outside agencies and uses such information in the completion of the assessment and care planning
• Coordinates, identifies, and/or initiates significant change MDS
• Maintains PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator
• Remains current with AANAC requirements
