Position Summary

The MDS Coordinator will assist the Director of Nursing in ensuring documentation compliance with federal, state, and certification guidelines. Responsibilities include coordinating the RAI process, ensuring timely and complete MDS, CAAS, and Care Plans.

Qualifications

• Current LVN license
• Preferred: Minimum of two years nursing experience in a skilled nursing facility
• Knowledge of RAI Process, Medicare PPS, Medicaid reimbursement, OBRA regulations, and Minimum Data Set

Essential Functions

• Ensure resident safety and compliance with regulations
• Participate in surveys and correct deficiencies
• Manage the RAI process and documentation
• Coordinate assessments, care plans, and interdisciplinary communication
• Maintain confidentiality and timely submission of assessments
• Support staff training on RAI and related processes

Physical Requirements

Ability to perform movements such as pushing, pulling, lifting up to 50 pounds with or without accommodations.

Work Schedule

As assigned, including some weekends, evenings, and holidays; non-exempt position.
