POSITION SUMMARY:

The MDS Coordinator assists the Director of Nursing in ensuring documentation compliance with federal, state, and certification guidelines. Coordinates the RAI process, ensuring the timeliness and completeness of MDS, CAAS, and Care Plans.

QUALIFICATIONS:

• Current LVN license
• Minimum of two years nursing experience in a skilled nursing facility (preferred)
• Knowledge of RAI Process, Medicare PPS, Medicaid reimbursement
• Understanding of Quality Indicator process
• Knowledge of OBRA regulations and MDS
• Experience with care plan processes

ESSENTIAL FUNCTIONS:

• Ensure resident safety
• Assist in regulatory adherence and survey processes
• Participate in audits and report trends
• Manage the RAI process from MDS to care plans
• Coordinate assessments and documentation
• Facilitate communication among staff and residents
• Maintain confidentiality and documentation standards
• Monitor compliance and participate in team meetings
• Support staff training on RAI and PPS processes
• Ensure timely submission of assessments and data

PHYSICAL REQUIREMENTS:

Ability to perform movements such as pushing, pulling, lifting up to 50 pounds with or without accommodations.

WORK SCHEDULE:

As assigned, including some weekends, evenings, and holidays; non-exempt position.
