Westwood Post Acute is seeking an experienced MDS Nurse to join our team in a hybrid role with onsite and remote elements. The position focuses on MDS documentation, data accuracy, and chart maintenance for residents.

Responsibilities include collecting MDS documents within required timeframes, reconciling data against original MDS forms, filing documents in charts, and upholding confidentiality. LPN license in Colorado and MDS 3.0 experience are required.

Competenze

• Active LPN license in Colorado.
• Experience as an MDS Coordinator utilizing MDS 3.0.
• Experience in a SNF environment.

Rate Range:

$40-$44/hour
