Job summary

Skilled Nursing Facility in Dayton, OH seeks an experienced MDS Coordinator to lead MDS processes in a facility setting. This role is ideal for an LPN or RN with strong long-term care experience who understands MDS, reimbursement, and regulatory requirements.

The successful candidate will coordinate assessments, support care planning, ensure documentation accuracy, and collaborate with nursing, therapy, social services, and other departments to drive quality and compliance.

Qualifications

• Current Ohio LPN or RN license.
• Experience with MDS in skilled nursing or long-term care preferred.
• Knowledge of MDS 3.0, RAI guidelines, Medicare/Medicaid, and PDPM.
• PointClickCare experience preferred.
• RAC-CT certification is a plus.
• Strong organizational skills and attention to detail.
