Job summary

Qualifications

• Must possess teamwork skills to contribute to and participate in team concepts.
• Must demonstrate ability to interview residents and collect information for assessments.
• Must be able to enter data into computer programs for minimum data set assessments.

Responsibilities

• Assess residents’ physical and mental function and document in minimum data set forms.
• Determine referrals to other health care professionals as needed.
• Complete minimum data set assessments and related reports with accuracy.
• Participate in the Quality Assurance program and interdisciplinary team meetings.
• Coordinate plan of care and care conference meetings.
• Reconcile end of month/end of quarter reports and communicate findings to the team.
