Job summary

Phoenix Health Services in Akron, OH is seeking a qualified nurse to assess residents’ physical and mental function and document MDS forms completely and accurately. You will determine referrals and use the resident assessment protocols to guide care decisions.

Collaborate with administration, nursing staff, physicians, and other disciplines; participate in Quality Assurance activities and interdisciplinary meetings; ensure timely completion of assessments and CMS reporting in the state system.

Qualifications

• Must possess a current, unencumbered, active license as LPN or RN in the state.
• Ability to interview residents, residents’ representatives and others for MDS assessment.
• Experience entering data into the computer program for MDS assessments.
• Ability to print the MDS assessments and related CMS reports.
• Strong teamwork and ability to participate in QAPI and interdisciplinary meetings.
• Ability to reconcile end-of-month/quarter reports.
• Knowledge of nursing practices and CMS guidelines.

Responsibilities

• Assess residents’ physical and mental function and document MDS forms.
• Determine referrals to other health care professionals based on assessment.
• Coordinate care plans and participate in care conference meetings.
• Participate in Quality Assurance program and interdisciplinary team meetings.
• Maintain open communication with nursing staff, residents, families and other stakeholders.
• Adhere to CMS timelines and regulatory reporting requirements.
