Lexington Country Place, part of Five Star Senior Living Inc., is seeking a dependable MDS LPN to join our nursing team. This role coordinates and completes MDS assessments, supports accurate clinical documentation, and works with the interdisciplinary team to ensure quality resident care and regulatory compliance.

Qualifications

• Current LPN/LVN license in the state of employment.
• Long-term care or skilled-nursing experience preferred.
• MDS experience preferred, but training may be available for the right candidate.
• Strong organizational, communication, and documentation skills.

Responsibilities

• Complete and coordinate MDS assessments accurately and within required deadlines.
• Review clinical records and documentation to support accurate MDS coding.
• Collaborate with nursing, therapy, dietary, social services, and other departments.
• Participate in care planning and interdisciplinary team meetings.
• Monitor resident changes and ensure appropriate documentation.
• Assist with Medicare/Medicaid documentation and regulatory requirements.
• Maintain compliance with applicable CMS, state, and facility requirements.
