Phoenix Senior Living in South Carolina is seeking a dependable MDS LPN to join Lexington Country Place. The role coordinates MDS assessments, supports accurate clinical documentation, and works with the interdisciplinary team to ensure quality resident care and regulatory compliance.

The ideal candidate will hold an LPN/LVN license in the state, have prior long-term-care or skilled-nursing experience, and be strong in organization and communication. Training on MDS may be available.

Qualifications

• Current LPN/LVN license in the state of employment.
• Long-term-care or skilled-nursing experience preferred.
• MDS experience preferred; training may be available.
• Strong organizational, communication, and documentation skills.
• Detail-oriented with the ability to meet deadlines.
• Ability to work effectively as part of an interdisciplinary team.

Responsibilities

• Complete and coordinate MDS assessments accurately and within required deadlines.
• Review clinical records to support accurate MDS coding.
• Collaborate with nursing, therapy, dietary, social services, and other departments.
• Participate in care planning and interdisciplinary team meetings.
• Monitor resident changes and ensure appropriate documentation.
• Assist with Medicare/Medicaid documentation and regulatory requirements.
• Maintain compliance with CMS, state, and facility requirements.
