Fletcher Rehabilitation and Healthcare Center is seeking a highly skilled MDS Hybrid Position to join our team as a Hybrid MDS Coordinator. This full-time role blends onsite and remote work (3 days onsite, 2 days remote) to coordinate and manage nursing care plans for our residents.

You will ensure accurate MDS documentation, maintain regulatory compliance, audit records, and support discharge processes. Required: current state nursing license and CPR certification; preferred: 1 year in

Qualifications

• Knowledge of MDS process and regulatory requirements.
• RN license active in state and CPR certification.
• Experience coordinating care plans in long-term care is preferred.

Responsibilities

• Keep abreast of federal and state regulations and standards.
• Develop and coordinate comprehensive care plans with the MDS.
• Manage and monitor the written plan of care for each resident.
• Monitor resident status changes and involve the clinical team as needed.
• Ensure MDS documentation meets regulatory and auditor requirements.
• Perform regular audits of documentation for accuracy.
• Assist with discharge processes and necessary forms.
• Maintain infection control practices and safety precautions.
• Maintain confidentiality of residents' health information.
• Report hazardous conditions and supply issues.

Job Summary:

We are seeking a highly skilled and experienced MDS Hybrid Position to join our team at Fletcher Rehabilitation and Healthcare Center. As a Hybrid MDS Coordinator, you will play a crucial role in the coordination and management of our nursing care plans, ensuring that our residents receive the highest level of care possible. This is a full-time position, with a blend of onsite and remote work (3 days onsite, 2 days remote).

Essential Functions:

• Keep abreast of current federal and state regulations, as well as professional standards.
• Assist with the development of comprehensive care plans for Residents in coordination with the MDS that accurately addresses the needs of the Resident.
• Coordinate, manage and monitor the written plan of care for each Resident of the facility that identifies the needs of the Resident and goals to be accomplished for each need.
• Monitor Resident status changes to ensure appropriate and timely nursing or clinical team involvement.
• Assure MDS and support documentation are accurate representations of the Resident and meet regulatory and auditor requirements.
• Perform regular audits of documentation to assure accuracy.
• Assist nursing management with the discharge process.
• Perform administrative requirements, such as completing necessary forms and reports.
• Assure that established infection control and standard precaution practices are maintained when providing care.
• Follow established safety precautions when performing tasks and using equipment and supplies.
• Maintain strict confidentiality regarding sensitive health information of Residents.
• Reports all hazardous conditions, damaged equipment and supply issues to appropriate persons.

Required Education and Experience:

• Current State License as a Nurse
• C.P.R. Certified

Preferred Education and Experience:

• One year experience as a Nurse in a long-term care setting
