Job summary

Fletcher Rehabilitation and Healthcare Center is seeking an experienced MDS Hybrid Coordinator to oversee nursing care plans and MDS processes. The role combines on-site and remote work, three days at the facility and two days remote, to ensure the highest level of resident care.

The ideal candidate holds an RN license with CPR certification and at least one year of long-term care nursing experience, plus strong knowledge of MDS processes and regulatory standards.

Qualifications

• Current State RN license.
• CPR certification.
• One year experience as a nurse in a long-term care setting is preferred.
• Knowledge and training in all aspects of the MDS process.

Responsibilities

• Keep abreast of current federal and state regulations and professional standards.
• Assist with development of comprehensive care plans in coordination with the MDS.
• Coordinate, manage and monitor the written plan of care for each resident.
• Monitor resident status changes and ensure timely nursing or clinical involvement.
• Ensure MDS documentation meets regulatory and auditor requirements.
• Perform regular audits of documentation for accuracy.
• Assist nursing management with discharge processes.
• Complete necessary forms and reports.
• Maintain infection control and standard precautions in care.
• Follow safety precautions when using equipment and supplies.
• Maintain confidentiality of resident health information.
• Report hazardous conditions, damaged equipment and supply issues.

Job Summary

We are seeking a highly skilled and experienced MDS Hybrid Position to join our team at Fletcher Rehabilitation and Healthcare Center. As a Hybrid MDS Coordinator, you will play a crucial role in the coordination and management of our nursing care plans, ensuring that our residents receive the highest level of care possible. This is a full-time position, with a blend of onsite and remote work (3 days onsite, 2 days remote).

Essential Functions

• Keep abreast of current federal and state regulations, as well as professional standards.
• Assist with the development of comprehensive care plans for Residents in coordination with the MDS that accurately addresses the needs of the Resident.
• Coordinate, manage and monitor the written plan of care for each Resident of the facility that identifies the needs of the Resident and goals to be accomplished for each need.
• Monitor Resident status changes to ensure appropriate and timely nursing or clinical team involvement.
• Assure MDS and support documentation are accurate representations of the Resident and meet regulatory and auditor requirements.
• Perform regular audits of documentation to assure accuracy.
• Assist nursing management with the discharge process.
• Perform administrative requirements, such as completing necessary forms and reports.
• Assure that established infection control and standard precaution practices are maintained when providing care.
• Follow established safety precautions when performing tasks and using equipment and supplies.
• Maintain strict confidentiality regarding sensitive health information of Residents.
• Reports all hazardous conditions, damaged equipment and supply issues to appropriate persons.

Required Education and Experience

• Current State License as a Nurse
• C.P.R. Certified

Preferred Education and Experience

• One year experience as a Nurse in a long-term care setting
