Job summary

Fusion Healthcare Rehabilitation and Nursing Center in Massachusetts seeks a dedicated RN with MA license to manage MDS and care plans in a long-term care setting. You will assess, initiate care plans, and maintain accurate records, while collaborating with a team in a family-friendly environment. Daily pay option available to support staff needs.

The ideal candidate is organized, highly motivated, and has strong communication skills. Proficiency with computers and documentation is essential.

Qualifications

• Registered nurse with valid MA license required.
• MDS experience is a must.
• Experience in long-term care settings preferred.
• Strong organizational and communication skills.
• Proficient with basic computer systems and documentation.

Responsibilities

• Perform accurate assessments and participate in MDS and care plan development.
• Initiate and support care plans as assigned.
• Create and distribute monthly care plan calendars.
• Maintain and update care plans and assessments.
• Monitor and audit clinical records for accuracy and timeliness.
• Protect resident and facility confidentiality.

Duties and Responsibilities:

• Completing accurate assessments, MDS & care plans as assigned
• Initiating care plans and supporting activities as assigned
• Creating and distributing monthly care plan calendars in a timely fashion
• Maintaining & updating all care plans and assessments as required
• Monitoring & auditing clinical records, ensuring accuracy & timeliness
• Informing DON of persistent issues related to non-compliant documentation
• Protecting the confidentiality of Resident & Facility information at all times

Requirements:

• Valid RN License
• MUST HAVE PRIOR MDS EXPERIENCE
• Long Term Care Experience Required
• Must be highly organized, professional & motivated
• Should have solid computer skills
• Excellent communication skills
• Should be friendly and a team worker

Experience:

• MDS: 2 years (Required)
• long term care: 2 years (Required)
