Laurel Wellness and Nursing Center in Fontana, CA is seeking an MDS Coordinator to join our team. The MDS Coordinator completes MDS for all residents according to OBRA and PPS guidelines, supporting high quality nursing care.

Minimum requirements include a California RN or LVN license, 1–2 years MDS experience or MDS completed course, BLS/CPR, and strong math and regulatory knowledge. Starting pay is $45 per hour with opportunities for advancement based on experience.

Responsibilities

• Participate in admission process to assess nursing needs and placement.
• Determine potential RUGs and related admission expenses.
• Review applications and communicate assessment results to Admissions Coordinator.
• Complete and assure the accuracy of the MDS process for all residents.
• Maintain knowledge of Medicare criteria and inform nursing staff of changes.
• Monitor Case Mix Index (CMI) scores for Medicaid reimbursement risk.
• Facilitate problem-solving for complex admissions.
• Monitor Medicare assessment schedules and nursing documentation for accuracy.
• Adhere to attendance policy and maintain reliable presence.
• Administer medications per physician orders.
• Respond promptly to call lights and provide interventions as needed.
• Pain management and interventions as required.
• Assess behavior and provide interventions as indicated.
• Wound and therapy treatments as prescribed.
• Document and follow-up with physician and family.

We are looking for an MDS Coordinator to join our team! The MDS Coordinator completes MDS for all residents according to OBRA and PPS guidelines. Assists in providing a clean, safe, happy and healthy environment for residents by performing the duties as described below. The goal is to promote the patient’s well-being by providing high quality nursing care.

• Valid and current California RN or LVN License
• 1-2 years MDS experience or MDS completed course
• BLS or CPR (required)
• Fully vaccinated against COVID-19 including Booster shots
• 1-2 years experience in MDS coordinator role or related experience required
• Exceptional mathematical skills and ability to apply them to tasks such as analysis of variance and sampling theory
• Deep medical terminology knowledge
• Experience in a clinical and healthcare setting preferred
• Excellent leadership skills
• Demonstrable knowledge of state and federal regulations
• Previous experience in a SNF or similar job setting would be preferred
• Excellent knowledge of nursing care methods and procedures.
• Excellent knowledge of emergency care.
• In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times
• A team player with excellent communication and interpersonal skills

Primary MDS Coordinator Responsibilities

• Participate in the admission process of prospective residents in terms of their nursing needs and appropriate placement
• Determine potential Resource Utilization Groups (RUGs) and expense associated with a potential admission
• Review all applications for admission, followed by on-site or phone assessment and communicate results of assessment to the Admissions Coordinator
• Complete and assure the accuracy of the MDS process for all residents
• Maintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicate changes in regulations
• Monitor Case Mix Index (CMI) scores, looking for potential risks and/or changes that may affect Medicaid reimbursement
• Facilitate problem‑solving for complicated admissions
• Monitor Medicare assessment schedules and nursing documentation to ensure accuracy and timely submission
• Complies with the facility’s attendance policy requiring regular, reliable, and punctual attendance

Other Responsibilities

• Adjust and administer patient’s medication and provide treatments according to physician’s orders
• Ensures timely response to call lights

• Assess behavior and provide interventions as indicated

• Patient complaints / concerns
• Identifies/assesses emergency medical situations and provides appropriate interventions as indicated
• Documentation and follow‑up with timely notification of physician and family
• Care planning as needed
• Performs other duties or functions as assigned by the DON and/or Administrator

PAY SCALE:

Depending on a number of factors, including, but not limited to location, regularly scheduled work shift, knowledge, skills, experience, and expertise.
