Job summary

Ciena Healthcare is seeking an experienced MDS Coordinator (RN) to oversee the RAI process and coordinate care planning. You will supervise the Care Management Nurse and MDS Nurse, ensuring timely MDS completion and accurate PPS alignment.

The ideal candidate has RN licensure, RAI knowledge, and prior MDS nurse experience, with a passion for teamwork and resident-focused care.

Qualifications

• Registered Nurse (RN) with active licensure.
• Knowledge of the Resident Assessment Instrument (RAI) process and PPS.
• Experience as an MDS Nurse or coordinator preferred.

Responsibilities

• Complete the MDS, CAAs and care plans within regulatory timeframes.
• Coordinate scheduling of the RAI process with the interdisciplinary team.
• Assess residents through physical assessment, interview and chart review.
• Discuss resident care needs with care givers, physicians, nursing, social services, therapy, dietary and activity staff.
• Review hospital data and outside information for care planning and assessments.
• Identify and initiate significant changes in MDS where needed.
• Prepare for PPS meetings and maintain PPS board for Medicare days and RUGs in coordination with the Care Management Coordinator.
• Remain current with AANAC requirements.

Responsibilities

• Completes the MDS, CAA's and care plans within regulated time frames.
• Coordinates scheduling the RAI process with the interdisciplinary team
• Assesses resident through physical assessment, interview and chart review.
• Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.
• Reviews information from hospital, consults and outside agencies and uses such information in the completion of the assessment and care planning.
• Coordinates, identifies, and/or initiates significant change MDS'
• Is prepared to conduct PPS meetings maintaining MDS assessments per Medicare schedule and maintain PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator
• Remains current with American Association of Nursing Assessment Coordinators (AANAC) requirements.

• Experience as an MDS Nurse
