Willowbrook Manor in Flint, Michigan, is seeking an experienced MDS nurse to stay clinically involved in patient care without bedside duties. You will work autonomously on a collaborative team that values communication and efficiency.

You will complete MDS, CAA’s and care plans within required timeframes, assess residents, discuss needs with physicians and staff, and ensure PPS meetings and Medicare scheduling are followed.

Qualifikationen

• Registered Nurse (RN) or Licensed Practical Nurse (LPN).
• Knowledge of the Resident Assessment Instrument (RAI) and PPS processes preferred.
• Experience as an MDS Nurse.

• Completes the MDS, CAA’s and care plans within regulated time frames.
• Assesses residents through physical assessment, interview and chart review.
• Discusses resident care needs with care givers, including physicians, nursing, social services, therapy, dietary, and activity staff.
• Reviews information from hospital, consults and outside agencies and uses such information in the completion of the assessment and care planning.
• Coordinates, identifies, and/or initiates significant change MDS.
• Prepared to conduct PPS meetings, maintain MDS assessments per Medicare schedule, and monitor PPS board in absence of the Care Management Coordinator.

Responsibilities:

• Completes the MDS, CAA’s and care plans within regulated time frames.
• Assesses resident through physical assessment, interview and chart review.
• Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.
• Reviews information from hospital, consults and outside agencies and uses such information in the completion of the assessment and care planning.
• Coordinates, identifies, and/or initiates significant change MDS’
• Is prepared to conduct PPS meetings maintaining MDS assessments per Medicare schedule and maintains PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator
• Remains current with the American Association of Nursing Assessment Coordinators (AANAC) requirements.

Qualifications:

• Registered Nurse, RN or Licensed Practical Nurse
• Knowledge of the Resident Assessment Instrument (RAI) process, including the principles the Prospective Payment Process (PPS) strongly preferred.
• Experience as an MDS Nurse
