Qualifications

• Two years of MDS experience preferred, but not required.
• Current RN license in the state of employment.
• Working knowledge of the MDS 3.0.

Responsibilities

• Coordinate the RAI Process for accuracy and compliance.
• Collaborate with the interdisciplinary team to plan care.
• Conduct Care Plan conferences with patients, residents, and families.
• Provide education related to the RAI Process and accurate coding.
• Monitor Quality Measures and ensure MDS accuracy reflects standards.
• Maintain accurate documentation and timely submissions to state databases.
• Ensure compliance with Medicare and Medicaid guidelines.
