Diversicare is seeking an MDS Coordinator (RNAC) to join our Birmingham team. You will oversee RAI processes, ensure coding accuracy, and drive quality measures in collaboration with the care team.

Qualifications

• Two years of MDS experience preferred, but not required.
• Current RN license in the state of employment.
• Working knowledge of the MDS 3.0.

Responsibilities

• Coordinate the RAI Process with compliance.
• Collaborate with interdisciplinary team to plan care.
• Conduct Care Plan conferences with patients and families.
• Provide education on the RAI process and MDS coding accuracy.
• Monitor Quality Measures and ensure MDS accuracy.
• Maintain documentation and timely submissions to state databases.
• Ensure compliance with Medicare/Medicaid guidelines.
