Job summary

Atlas Healthcare, LLC in Massachusetts seeks an experienced MDS Coordinator (RN) to ensure accurate and timely completion of clinical assessments for residents. You will support an interdisciplinary team and drive quality improvement through effective MDS processes.

Responsibilities include leading care conferences, performing comprehensive resident assessments, completing MDS with RAPS, and ensuring regulatory compliance.

Qualifications

• RN licensed in Massachusetts.
• Minimum of three years in geriatric nursing or rehabilitation preferred.
• CPR-certified.
• Fluent in reading, writing and understanding English.
• Proficient with MS Office and standard office equipment.
• Ability to work scheduled hours as required.

Responsibilities

• Maintain care conferences calendar and conduct interdisciplinary meetings as needed.
• Conduct comprehensive assessments of residents through medical record review, interviews, and resident evaluation.
• Complete MDS with RAPS and resident summaries; develop resident care plans.
• Ensure timely completion of all aspects of the RAI process and transmission per regulations.
• Lead case management for Managed Care when applicable.
• Participate in CMI and billing meetings.
• Communicate PDPM recommendations as appropriate.
• Review final validation reports from the State and make corrections as needed.
• Participate in weekly Medicare meetings and communicate pertinent MDS information.
• Review Medicare resident for appropriateness of Medicare coverage.
• Obtain monthly Quality Indicator Reports and provide to nursing management.
• Perform other duties as directed by management.
