We're a skilled nursing and rehabilitation community offering the full continuum of care — short-term rehab, long-term care, memory care, and assisted living — all under one roof, with a team known for personalized care plans and a genuine community feel. As our MDS Coordinator, you'll play a central role in making sure every resident's care and our facility's reimbursement are captured accurately and on time.
We're looking for an MDS Coordinator with real Minimum Data Set (MDS) experience — this is not a training position. Prior nursing home experience is required. If you're an MDS Coordinator who knows the Resident Assessment Instrument (RAI) process, understands the Patient-Driven Payment Model (PDPM), and takes pride in accurate, timely assessments, we'd love to talk with you.
Coordinate and complete Minimum Data Set (MDS) assessments accurately and on schedule
Manage the Resident Assessment Instrument (RAI) process, including care plan coordination
Ensure documentation supports accurate reimbursement under the Patient-Driven Payment Model (PDPM)
Work closely with nursing, therapy, and the interdisciplinary team on assessments and care planning
Monitor assessment schedules and transmission deadlines to stay survey-ready
Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Ohio
Prior experience as an MDS Coordinator or MDS Nurse — Minimum Data Set (MDS) experience is required
Must have experience in a skilled nursing facility (SNF) / long-term care (LTC) setting
Supportive clinical leadership and an experienced interdisciplinary team
A stable, full-continuum community — skilled nursing, rehab, memory care, and assisted living on one campus
A workplace where staff and residents know each other by name
