Job summary

CareCore at Mentor seeks an experienced MDS Coordinator to oversee the RAI process and support accurate clinical reimbursement while upholding high standards of resident care.

You will coordinate MDS assessments, collaborate with an interdisciplinary team, and ensure timely, compliant documentation, contributing to quality outcomes and regulatory readiness.

Qualifications

• MDS experience in skilled nursing or long-term care settings is required.
• Must hold an active Ohio RN or LPN license; RAC-CT preferred.
• Strong knowledge of CMS regulations, reimbursement, and PDPM guidelines.
• Excellent assessment, documentation, and organizational abilities.

Responsibilities

• Coordinate and complete MDS assessments per federal/state regulations.
• Ensure timely, accurate completion of the RAI process.
• Collaborate with nursing, therapy, dietary, social services, and others.
• Monitor Medicare/Medicaid documentation to support reimbursement.
• Participate in care plan meetings and interdisciplinary conferences.
• Review clinical documentation for accuracy and compliance.
• Support quality improvement initiatives and regulatory readiness.
• Maintain current CMS knowledge and reimbursement guidelines.

Are you an experienced MDS professional looking to join a supportive, resident-focused skilled nursing team? We are seeking an organized, detail-oriented MDS Coordinator to oversee the Resident Assessment Instrument (RAI) process and help ensure accurate clinical reimbursement while supporting quality resident care.

• Coordinate and complete MDS assessments in accordance with federal and state regulations.

• Ensure timely and accurate completion of the RAI process.
