Job summary

Regalcare of Holyoke is seeking an experienced MDS Coordinator to join our leadership team in a full-time role. Coordinate and complete MDS assessments in compliance with state and federal regulations while supporting interdisciplinary care planning.

You will manage the RAI process, ensure documentation supports outcomes and reimbursement, and collaborate with nursing, therapy, and administrative teams. Strong organizational skills and a good standing RN/LPN license are required.

Qualifications

• Active RN or LPN license in good standing.
• Experience with MDS assessments in long-term care (preferred).
• Strong knowledge of Medicare/Medicaid guidelines and RAI process.
• Strong organizational and time-management skills.
• Ability to work independently and meet deadlines.

Responsibilities

• Coordinate and complete accurate, timely MDS assessments in compliance with state and federal regulations
• Manage the RAI process and support interdisciplinary care planning
• Ensure documentation supports clinical outcomes and reimbursement
• Collaborate with nursing, therapy, and administrative teams
• Monitor assessment schedules, audits, and quality measures
• Support surveys and quality improvement initiatives
