Job summary

The Springs at Rochester Hills seeks an experienced MDS Coordinator (RN or LPN) to join our skilled nursing team. This role focuses on coordinating the Resident Assessment Instrument (RAI) and MDS process to ensure accurate assessments and regulatory compliance.

The ideal candidate will have Michigan RN or LPN licensure, prior MDS experience in a skilled nursing facility, and strong collaboration with interdisciplinary teams to develop resident-centered care plans and support compliant

Qualifications

• Current, active Michigan RN or LPN license.
• Previous MDS experience in a skilled nursing facility is required.
• Working knowledge of the RAI process, care planning, and skilled nursing regulations.
• Strong clinical assessment and documentation skills.
• Excellent organization, communication, and time-management abilities.

Responsibilities

• Coordinate and complete accurate, timely MDS assessments
• Manage the RAI process and assessment calendar
• Lead interdisciplinary care plan meetings
• Ensure care plans reflect residents’ current needs, goals, and services
• Review clinical documentation for accuracy, completeness, and regulatory compliance
• Monitor changes in resident condition and ensure appropriate assessments are completed
• Collaborate with nursing, therapy, social services, dietary, and other departments
• Support accurate reimbursement through proper assessment and documentation
• Stay current with CMS guidelines, Medicare requirements, and MDS updates
• Assist with audits, surveys, and quality-improvement initiatives
