Job summary

The Springs at Rochester Hills is seeking an experienced MDS Coordinator (RN or LPN) to join our skilled nursing team, coordinating and completing the RAI and MDS processes in line with federal and state regulations. This role collaborates with nursing, therapy, dietary, social services and other team members.

The ideal candidate holds an active RN or LPN license and has prior MDS experience in a skilled nursing facility, with strong assessment, documentation, and interdisciplinary leadership to

Qualifications

• Current, active Michigan RN or LPN license.
• Previous MDS experience in a skilled nursing facility is required.
• Working knowledge of the RAI process, care planning, and skilled nursing regulations.

Responsibilities

• Coordinate and complete accurate, timely MDS assessments
• Manage the RAI process and assessment calendar
• Lead interdisciplinary care plan meetings
• Ensure care plans reflect residents’ current needs, goals, and services
• Review clinical documentation for accuracy, completeness, and regulatory compliance
• Monitor changes in resident condition and ensure appropriate assessments are completed
• Collaborate with nursing, therapy, social services, dietary, and other departments
• Support accurate reimbursement through proper assessment and documentation
• Stay current with CMS guidelines, Medicare requirements, and MDS updates
• Assist with audits, surveys, and quality-improvement initiatives
