Job summary

iCare Health Network is seeking a dedicated MDS Coordinator to oversee the Medicare/Medicaid case-mix documentation and care assessments, ensuring accurate reimbursement. The role integrates input from Nursing, Dietary, Social Services and Rehabilitation teams to optimize RUG category achievement.

The position offers a Monday–Friday schedule with flexible hours, starting at $65,000 per year, with potential for higher compensation in RN/LPN pathways as detailed in the benefits section.

Qualifications

• Active, unemcumbered nursing license (RN or LPN) in MA required.
• Quality Assurance/Improvement knowledge required.
• Experience in supervision in long-term care preferred.

Responsibilities

• Coordinate assessment, planning, implementation and evaluation of resident care per MDS process.
• Facilitate interdisciplinary care conferences.
• Coordinate, complete and review Minimum Data Sets (MDS).
• Educate and train nursing staff and other departments on MDS roles and processes.
• Ensure current MDS/PPS updates are implemented and staff are trained.
• Cross-train between PPS and OBRA.
• Conduct regular resident interviews as required by MDS.
• Respond to clinical emergencies, regulatory visits, surveys and issues outside of normal hours.
