Sterling Healthcare Management is seeking an experienced MDS Coordinator to lead the RAI process and optimize PDPM reimbursement in a skilled nursing setting. You will partner with nursing, therapy, dietary, and social services to ensure comprehensive assessments and individualized care plans.
The role requires strong MDS coordination, PDPM knowledge, and proficiency with iQIES and common EHR systems such as PointClickCare or MatrixCare; RN license preferred.
We are looking for a detail-oriented, experienced MDS Coordinator to lead our RAI process and drive accurate PDPM reimbursement. You will work closely with nursing, therapy, dietary, and social services to ensure assessments are complete, care plans are individualized, and our facility performs at its best. RN license strongly preferred — LPN with significant MDS experience will be considered.
