The MDS Coordinator is responsible for supervising and coordinating all MDS (Minimum Data Sets) and care plans for residents, ensuring compliance with state requirements. Responsibilities include:

1. Monitoring resident care during the MDS process and reporting concerns to leadership
2. Assisting with pre-admission screening to estimate resident’s RUGs group
3. Reporting contraindications between resident status and documentation

The ideal candidate will:

1. Coordinate and conduct MDS and Care Plans per facility policies
2. Notify care team members when care plans are due and ensure completion
3. Develop or revise resident care plans quarterly and with condition changes
4. Complete all required PPS and MDS assessments
5. Ensure all MDS, CATs, and CAAS are properly completed
6. Coordinate care plan meetings and encourage team participation
7. Submit MDS to agencies accurately and timely
8. Maintain Resident Assessment and Care Plan Schedule
9. Observe residents’ rights regarding confidentiality and privacy
10. Maintain effective communication among departments for current care plans
