Position: MDS Coordinator

As an MDS Coordinator at The Springs of Red Oak, your primary responsibility is to promote the physical and emotional well-being of our residents. You'll utilize the Resident Assessment Instrument (RAI) to gather vital information during initial and periodic interviews with residents and their families. Using the Minimum Data Set (MDS) as your primary tool, you'll assess residents' functional capabilities and develop individualized care plans to ensure optimal care.

Key Responsibilities:

- Gather information using the RAI during resident interviews.

- Utilize the MDS to assess residents' functional capabilities.

- Develop and implement individualized care plans.

- Ensure compliance with medical and ethical standards.

- Manage MCR/Managed Care MDS updates and care plans.
