Description

The MDS Coordinator is responsible for reviewing and preparing for new admissions according to established policies and procedures. The incumbent will also maintain relations with community partners, family members, and personnel.

Responsibilities for the MDS Coordinator:

1. Assess and determine the health status and level of care of all new admissions.
2. Ensure accurate and timely completion of all MDS Assessments, including PPS, Medicare, quarterly, annual, and significant change assessments.
3. Communicate level of care for new residents to all disciplines.
4. Coordinate interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission, ensuring documentation is complete and thorough as mandated by federal, state, and medical standards.
5. Responsible for data entry to ensure accurate data entry and electronic submission of MDS assessments.
6. Verify electronic submissions of MDS, perform corrections when necessary, and maintain records.
7. Educate IDT members and staff on MDS-related assessments and documentation.
8. Schedule and conduct resident care conferences in compliance with regulations, ensuring completion of MDS reviews prior to conferences.
9. Stay aware of recent and upcoming MDS changes.

Qualifications for the MDS Coordinator:

1. Must possess a current, unencumbered, active license to practice as an RN or LPN in Illinois.
2. At least one (5) year experience in rehabilitative and restorative nursing or as a staff nurse in a healthcare facility; experience in long-term care required.
3. Preferably an RN licensed in Illinois.
4. MDS Certification is required (experience a must).
