Job summary

Avantara Lincoln Park is a premier skilled nursing and rehabilitation facility in Chicago. We seek an RN to lead the MDS process and utilization management, ensuring compliant, person-centered care across short-term and long-term residents.

You will coordinate care plans, supervise nursing notes, and support discharge processes while participating in QAPI and compliance trainings. Strong knowledge of regulations and CPR certification are required.

Qualifications

• Graduate from an accredited nursing program.
• Valid, unencumbered Registered Nurse (RN) license in the state of practice.
• Knowledge of MDS process and regulatory requirements.
• CPR certification.

Responsibilities

• Lead management of the MDS and utilization management per current standards.
• Develop comprehensive care plans in coordination with the MDS.
• Coordinate and monitor the written plan of care for each resident.
• Assist nursing management with discharge processes and chart reviews.
• Attend departmental meetings and participate in QAPI initiatives.
