Bethesda Health Group is seeking an RN to manage MDS, admissions, and change in therapy across a long-term care setting in New Jersey. You will coordinate data entry, CMS reporting, and care plans with the interdisciplinary team to ensure timely, accurate assessments.

Qualifications

• Graduate of an accredited nursing program.
• Active RN license in the state you will be working.
• Two years experience with long-term care plans, MDS regulations, and CMS related to MDS preferred.
• Excellent communication skills with residents, visitors, and staff.
• Ability to manage time and organize daily schedule to meet productivity standards.
• Ability to stand and walk for long periods.
• Ability to lift heavy objects with proper technique and assistance as needed.
• Ability to document both electronically and manually.
• Ability to work independently with minimal supervision and be a team player.

Responsibilities

• Completes all MDS, admission, significant change, change of therapy, end of therapy, discharge and re-entry tracking forms per CMS timelines.
• Enters appropriate MDS data into a patient information system and transmits to CMS.
• Keeps MDS/CAA records available for a 15-month period.
• Collaborates with the interdisciplinary team for problem solving and care plan development.
• Develops and implements annual goals coordinating with team members.
