The position emphasizes quality care planning and cross-team collaboration. The ideal candidate holds a nursing degree with an active RN license, at least three years in long-term care, and MDS/RAI experience.

Qualifications

• RN license required.
• Minimum 3 years of clinical experience in long-term care.
• Prior MDS/RAI experience required.

Responsibilities

• Ensure timely and accurate completion of all MDS assessments per regulatory requirements.
• Supervise MDS data entry, validation, and transmission processes.
• Identify and resolve data discrepancies and validation issues.
• Prepare and present MDS-related reports to DON and leadership.
• Stay current on Medicare/Medicaid/RAI/MDS regulations and guidelines.
