Job summary

Journey in Kentucky is seeking a Professional registered nurse to coordinate MDS assessments and develop individualized care plans for residents. The role emphasizes compliance with Medicare/Medicaid and state regulations while collaborating with care teams to improve outcomes.

The MDS Coordinator will partner with nursing and therapy staff in a resident-centered model, ensuring timely updates and accurate documentation to support high-quality care.

Qualifications

• Must hold a current RN license in the state of employment.
• Working knowledge of the MDS 3.0 (current version of minimum data set).

Responsibilities

• Resident Assessment & Care Planning: Conduct and coordinate MDS assessments, develop individualized care plans, and ensure timely updates for significant changes in residents' conditions.
• Compliance & Documentation: Ensure accurate completion and submission of MDS assessments per Medicare/Medicaid guidelines and state/federal regulations.
• Interdisciplinary Collaboration: Facilitate care plan meetings, maintain cross-department communication, and support improvements in resident care outcomes.

Required Qualifications

The MDS Coordinator participates in the Resident Assessment Process through completion of the pertinent items on the Minimum Data Set (MDS) and the accompanying Care Area Assessments (CAAs). Participates in developing individualized resident Care Plans, identifying the needs of the residents and projected outcomes as required by Federal and State regulations.

Major Duties and Responsibilities:

• Compliance & Documentation: Ensure accurate completion and submission of MDS assessments in accordance with Medicare/Medicaid guidelines and state/federal regulations.
• Interdisciplinary Collaboration: Facilitate care plan meetings, maintain effective communication across departments, and support continuous improvements in resident care outcomes.
