Journey is seeking an MDS Coordinator to participate in the Resident Assessment Process by completing MDS items and CAAs, and to help develop individualized resident care plans in line with federal and state regulations.
The role requires a current RN license in the state of employment and strong knowledge of MDS 3.0, with collaboration across multidisciplinary teams to drive improved resident outcomes.
The MDS Coordinator participates in the Resident Assessment Process through completion of the pertinent items on the Minimum Data Set (MDS) and the accompanying Care Area Assessments (CAAs). Participates in developing individualized resident Care Plans, identifying the needs of the residents and projected outcomes as required by Federal and State regulations.
