Job summary

Complete Care - Connecticut is seeking a dedicated MDS Coordinator RN to support reimbursement and care planning within a long-term care setting.

This full-time day-shift role oversees MDS assessments, ensures timely completion, and coordinates care conferences with nursing leadership. An active CT RN license and LTC MDS experience are required.

Qualifications

• Active RN license in CT.
• MDS experience in a long-term care facility.
• Experience with Federal Medicare PPS reimbursement.

Responsibilities

• Monitor and guide the completion of resident assessments in a timely manner.
• Involvement and supervision of inputting MDS assessments.
• Transmit MDS assessments to the state and obtain receipt of validation.
• Schedule and facilitate care plan conferences on a quarterly and annual basis.
• Other duties as assigned by Director of Nursing and Administrator.
