Job summary

Lake City Center for Nursing and Healing, part of the Empire Care Centers family, is seeking an MDS Coordinator to oversee MDS assessments and ensure accurate, timely documentation in line with CMS and LTCI guidelines.

You will collaborate with nurses, therapists, and department heads to support care planning, analyze QM/QI data for performance improvement, and guarantee compliance with all MDS procedures and submission deadlines.

Qualifications

• Current Georgia RN license.
• Experience with MDS/RAI in long-term care preferred.
• Strong organizational and communication skills.
• Knowledge of CMS guidelines and care planning.
• Experience in long-term care setting.

Responsibilities

• Coordinate and oversee all MDS assessments per CMS/LTCI guidelines.
• Collaborate with nurses, therapists, and department heads for accurate documentation.
• Prepare and analyze QM/QI data to drive performance improvement.
• Ensure compliance with RAI and MDS procedures and submission deadlines.

What You’ll Do

• Coordinate and oversee all MDS (Minimum Data Set) assessments in accordance with CMS and LTCI guidelines

• Collaborate with nurses, therapists, and department heads to ensure accurate, timely documentation

• Prepare and analyze Quality Measure/Quality Indicator (QM/QI) data for performance improvement

• Ensure compliance with all RAI and MDS procedures and submission deadlines
