Job summary

Complete Care is seeking a Registered Nurse-RN-MDS Coordinator in New Jersey to lead accurate MDS assessments and care plan development. You will collaborate with nursing, therapy, social services and other departments to ensure quality resident care and regulatory compliance.

Ideal candidates hold a NJ RN license, have MDS coordination experience, and excel in clinical assessment, documentation, and team leadership in a skilled nursing setting.

Qualifications

• Associate's or Bachelor's Degree in Nursing.
• Current, unrestricted New Jersey RN license.
• Experience with MDS assessments in a skilled nursing setting preferred.
• Experience as an RN-MDS Coordinator or MDS assessment experience preferred.
• Knowledge of federal and state MDS regulations.
• Ability to work collaboratively with interdisciplinary teams.

Responsibilities

• Coordinate and complete accurate MDS assessments in accordance with federal and state guidelines.
• Partner with nursing, therapy, social services, and other departments to develop individualized resident care plans.
• Ensure documentation supports quality resident care, regulatory compliance, and reimbursement accuracy.
• Monitor assessment schedules and update MDS documentation as residents' conditions change.
• Participate in care plan meetings and contribute clinical expertise to improve resident outcomes.
• Educate and support clinical staff on MDS processes, documentation standards, and regulatory updates.
• Work closely with leadership to maintain compliance and promote continuous quality improvement.
• Help ensure Complete Care continues to provide the highest level of resident-centered care.

Registered Nurse-RN-MDS Coordinator Qualifications

• Associate's or Bachelor's Degree in Nursing

• Current, unrestricted New Jersey Registered Nurse (RN) license

• Previous long-term care experience preferred

• Experience as a Registered Nurse-RN-MDS Coordinator or MDS assessment experience in a skilled nursing setting preferred

• Strong clinical assessment, communication, and organizational skills

• Knowledge of federal and state MDS regulations

• Ability to work collaboratively with interdisciplinary teams
