Position Summary

As the

MDS Coordinator

Location: Lexington, KY

Job Type: Full-Time

Position Summary

As the MDS Coordinator, you will oversee the RAI process and ensure timely and accurate completion of MDS assessments in compliance with federal and state regulations, as well as company standards. You’ll play a critical role in driving clinical outcomes, optimizing reimbursement, and supporting resident-centered care.

Responsibilities

  • Coordinate and oversee the RAI (Resident Assessment Instrument) process for all residents.
  • Ensure timely and accurate completion of MDS assessments in accordance with federal, state, and company regulations.
  • Collaborate with the interdisciplinary team to support care planning and clinical excellence.
  • Assist with resident care needs as required.
  • Maintain up-to-date knowledge of clinical best practices and regulatory guidelines.
  • Address resident and family inquiries and concerns with professionalism and empathy.
  • Ensure clear and timely communication among residents, families, medical staff, administration, and regulatory agencies.
  • Understand the financial impact of MDS, including RUGs and case mix reimbursement.
  • Educate residents and families about services, procedures, and care planning in a clear and compassionate manner.


Qualifications

Required:

  • Active RN license in good standing (State of Kentucky or compact license).
  • Minimum 2 years of MDS/utilization review experience.
  • Strong understanding of long-term care regulations and MDS processes.
  • CPR certification (or ability to obtain during orientation; must remain current).


Preferred:

  • RAC-CT certification.
  • Experience in geriatrics, long-term care, or rehabilitation settings.
