MDS Coordinator

Alliance Health at Rosewood, a 135-bed non-profit skilled nursing care center in West Peabody, MA, is seeking an experienced MDS Coordinator. Join our growing team and make a difference in elder care. We are an agency-free facility with a collaborative, multidisciplinary approach.

The MDS Coordinator will be responsible for:

  1. Timely completion of MDS assessments, care plans, and CAAs.
  2. Supporting audits as required by the DNS.
  3. Attending morning meetings and QAPI sessions.
  4. Ensuring accurate C.N.A documentation.
  5. Staff education and training.
  6. Managing short-term PDPM processes.
  7. This position is in-person; remote work is not available.
  8. Must hold a current nursing license in MA (RN or LPN).
  9. This is a full-time position.
