Job summary

The Emerald Peek Rehabilitation and Nursing Center is seeking an experienced MDS Coordinator (RN) to join our clinical leadership team in a full-time role. The ideal candidate will manage MDS assessments, ensure regulatory compliance, and support optimal reimbursement while maintaining excellent resident care.

Responsibilities include coordinating the full MDS process, scheduling, documentation, and collaborating with interdisciplinary teams.

Qualifications

• Current and active RN license in the applicable state.
• Experience as an MDS Coordinator in SNF/Long-Term Care preferred.
• Strong knowledge of MDS, RAI, PDPM, Medicare/Medicaid and reimbursement processes.
• Experience with MDS software and electronic medical records.
• Excellent organizational, analytical, and time-management abilities.
• Ability to manage multiple deadlines and priorities.

Responsibilities

• Coordinate and complete MDS assessments accurately and within required timeframes.
• Manage the full MDS process, including scheduling, assessments, care area triggers, and documentation.
• Ensure CMS/state/federal regulatory compliance.
• Review clinical documentation to support accurate coding and reimbursement.
• Collaborate with nursing, therapy, social services, dietary and other teams.
• Participate in care plan meetings and reflect current resident needs.
• Monitor MDS schedules, deadlines, and validation reports.
• Identify documentation and coding opportunities and educate staff.
• Assist with Medicare, Medicaid and reimbursement processes.
• Stay current on MDS requirements, RAI, PDPM and related regulations.
• Participate in quality improvement and survey readiness.

Salary: Up to $130,000 per year

The Emerald Peek Rehabilitation and Nursing Center is seeking an experienced and detail-oriented MDS Coordinator (RN) to join our clinical leadership team.

This is an excellent opportunity for a Registered Nurse with strong MDS, clinical documentation, and reimbursement experience in a skilled nursing or long-term care setting. The MDS Coordinator will play a key role in ensuring accurate assessments, regulatory compliance, and optimal reimbursement while supporting exceptional resident care.

Responsibilities

• Coordinate and complete MDS assessments accurately and within required timeframes.

• Manage the full MDS process, including scheduling, assessments, care area triggers, and documentation.

• Ensure compliance with CMS, state, and federal regulations.

• Review clinical documentation to ensure accuracy and support appropriate coding and reimbursement.

• Collaborate with nursing, therapy, social services, dietary, and other interdisciplinary team members.

• Participate in care plan meetings and ensure resident assessments reflect current needs.

• Monitor MDS schedules, submission deadlines, and validation reports.

• Identify documentation and coding opportunities and provide education to clinical staff as needed.

• Assist with Medicare, Medicaid, and other reimbursement-related processes.

• Stay current on changes to MDS requirements, RAI guidelines, PDPM, and applicable regulations.

• Participate in quality improvement and survey readiness initiatives.

• Maintain accurate and organized records related to MDS assessments and submissions.

Qualifications

• Current and active Registered Nurse (RN) license in the applicable state.

• Previous experience as an MDS Coordinator in a Skilled Nursing Facility or Long-Term Care setting is strongly preferred.

• Strong knowledge of MDS, RAI, PDPM, Medicare, Medicaid, and reimbursement processes.

• Experience with MDS software and electronic medical records.

• Strong clinical assessment and documentation skills.

• Excellent organizational, analytical, and time-management abilities.

• Ability to manage multiple deadlines and priorities.

• Strong communication and collaboration skills.
