Complete Care at Monmouth is seeking a Registered Nurse-RN-MDS Coordinator to coordinate MDS assessments and develop individualized care plans with a collaborative interdisciplinary team.

The role emphasizes accurate documentation, regulatory compliance, and high-quality resident care, with a full-time day shift and opportunities for career growth within Complete Care at Monmouth.

Qualifications

• Associate's or Bachelor's Degree in Nursing.
• Current, unrestricted New Jersey RN license.
• Long-term care experience preferred.
• RN-MDS coordination or MDS assessment experience preferred.
• Strong clinical assessment, communication, and organizational skills; knowledge of MDS regulations.

Responsibilities

• Coordinate and complete accurate MDS assessments per federal/state guidelines.
• Work with nursing, therapy, social services and other departments to develop individualized resident care plans.
• Ensure documentation supports quality resident care, regulatory compliance, and reimbursement accuracy.
• Monitor assessment schedules and update MDS documentation as conditions change.
• Participate in care planning meetings and contribute clinical expertise to improve outcomes.
• Educate and support clinical staff on MDS processes and regulatory updates.

Registered Nurse-RN-MDS Coordinator Qualifications

• Associate's or Bachelor's Degree in Nursing
• Current, unrestricted New Jersey Registered Nurse (RN) license
• Previous long-term care experience preferred
• Experience as a Registered Nurse-RN-MDS Coordinator or MDS assessment experience in a skilled nursing setting preferred
• Strong clinical assessment, communication, and organizational skills
• Knowledge of federal and state MDS regulations
• Ability to work collaboratively with interdisciplinary teams

Registered Nurse-RN-MDS Coordinator Responsibilities

As a Registered Nurse-RN-MDS Coordinator, you will:

• Partner with nursing, therapy, social services, and other departments to develop individualized resident care plans.
• Ensure documentation supports quality resident care, regulatory compliance, and reimbursement accuracy.
• Monitor assessment schedules and update MDS documentation as residents' conditions change.
• Participate in care plan meetings and contribute clinical expertise to improve resident outcomes.
• Educate and support clinical staff on MDS processes, documentation standards, and regulatory updates.
• Work closely with leadership to maintain compliance and promote continuous quality improvement.
• Help ensure Complete Care at Monmouth continues to provide the highest level of resident-centered care.
