Job summary

Signature HealthCARE LLC is seeking a detail-oriented MDS Coordinator to join our Kentucky-based Rockcastle Health & Rehabilitation Center. You will collect data from medical records, conduct bedside assessments, and interface with families to complete the MDS accurately.

The ideal candidate is a licensed practical/nurse with at least 1 year of MDS experience and 3+ years of clinical healthcare experience, preferably in long-term care. Relevant PPS/Medicare knowledge is a plus.

Qualifications

• Active LPN/LVN license in state of Kentucky.
• Minimum 1 year MDS experience.
• Minimum 3 years clinical experience in healthcare; LTC preferred.

Responsibilities

• Collect information to complete the MDS from records and interviews.
• Participate in PPS, Medicare, and month-end meetings to ensure billing accuracy.
• Review Plan of Care quarterly and with Comprehensive Assessment updates.
