Emerald Healthcare in Lincoln, NE is seeking an experienced MDS Coordinator to ensure accurate resident assessments and regulatory compliance. You will act as in-house case manager coordinating with physicians, families, payers and staff to develop care plans.

The role requires an active RN license, a current CPR certification, and a minimum of two years of clinical experience, including one year in long-term care.

Qualifications

• Registered Nurse with active license in state of practice.
• Current CPR certification required.
• Minimum two (2) years of clinical experience in a health care setting.
• Minimum of one (1) year of experience in a long term care setting.
• Prior experience as an MDS coordinator with demonstrated expertise.

Responsabilités

• Accurate and thorough completion of MDS, CAAs and Care Plans per federal/state regulations and guidelines.
• Act as in-house Case Manager coordinating with physicians, families, payers and staff.
• Identify regulatory risk and communicate with CEO/DON about facility systems and clinical trends.
• Capture RUG score reflecting care and services provided.
• Understand MDS requirements for Medicare, Managed Care, and Medicaid.
• Ensure timely electronic submission of all MDS to state database and address validation reports.
• Lead Care Management Process with resident and family to resolve discharge barriers.
• Educate IDT members on RAI processes and train on updated procedures as needed.
• Analyze QI/QM data monthly with DON to identify trends.
• Ensure timely and accurate completion of Triple Check.
• Participate in committees as directed and adhere to Code of Conduct.
