Job summary

Skilled Nursing Care of Erie County in New York is seeking an experienced full-time MDS Coordinator to oversee the patient assessment process and ensure assessments and care plans are completed accurately and on time.

The ideal candidate will hold an active RN license in New York and RAC-CT MDS certification (or willingness to train). Knowledge of PDPM and OBRA regulations is required; on-site work is required though scheduling can be flexible.

Qualifications

• Active RN license from New York required.
• RAC-CT MDS certification and PDPM/OBRA knowledge preferred or trainable.

Responsibilities

• Oversee the patient assessment process from start to finish.
