Job summary

Champion Rehabilitation and Nursing Center seeks an experienced Registered Nurse with MDS expertise to join our long-term care team in Massachusetts. You will assess residents, develop and update care plans, and ensure accurate documentation in a supportive, family-friendly environment.

The role emphasizes timely care-plans, coordinated communication with the Director of Nursing, and maintaining resident confidentiality while providing high-quality care.

Qualifications

• Must hold active RN license in Massachusetts.
• Prior MDS experience required in long-term care.
• Strong organizational, communication, and teamwork skills.

Responsibilities

• Complete assessments, MDS and care plans as assigned.
• Initiate care plans and support activities.
• Create and distribute monthly care plan calendars on time.
• Maintain and update all care plans and assessments as required.
• Monitor and audit clinical records for accuracy and timeliness.
• Escalate non-compliant documentation to DON.

Duties and Responsibilities

• Completing accurate assessments, MDS & care plans as assigned
• Initiating care plans and supporting activities as assigned
• Creating and distributing monthly care plan calendars in a timely fashion
• Maintaining & updating all care plans and assessments as required
• Monitoring & auditing clinical records, ensuring accuracy & timeliness
• Informing DON of persistent issues related to non-compliant documentation
• Protecting the confidentiality of Resident & Facility information at all times

Requirements

• Valid RN License
• MUST HAVE PRIOR MDS EXPERIENCE
• Long Term Care Experience Required
• Must be highly organized, professional & motivated
• Should have solid computer skills
• Excellent communication skills
• Should be friendly and a team worker
